Performance Appraisal Request

Regular and structured performance appraisals are an integral tool to identify underperforming staff for training and performance management, as well as a retention and recognition process for employees that are meeting their expectations.

Problem

For many SME’s it is difficult to commit the time to collect employee self-reflection as well as their own comments on the KPIs set at the beginning of the period.  This self-reflection really sets the tone for a performance appraisal meeting and enables them to be conducted quickly and easily.

Solution

On-Demand HR has designed a simple form for managers and line managers to complete and following submission, the following will take place:

1) On Demand HR will review the KPI’s and consult with the business if required;

2) On Demand HR will send a document to the employee for the employee to:

a. Rate themselves between 1-5 on each KPI as well as adding free text comments

b. Give themselves an overall rating for the period and add free text comments regarding their overall performance

c. Digitally sign and date the document.

3)  After the employee has completed their portion, the document is then sent to the manager to provide ratings and free text comments.  This is typically done either ahead of or during the recommended performance appraisal meeting

4) The manager digitally signs off on their ratings & comments

5) A completed signed performance appraisal form is then sent for filing to:

a. The Employee

b. The Manager

c. On Demand HR
